How to use the service

This is a secure web-based application system. It is set up for Home Options Team and referral agencies to make referrals for people with housing related support needs.

It is very important that you include full contact details so that we can contact you if we need further information. If no specific named person's contact details who has assisted you are included the application will be rejected.

Before you start an application you will need to make sure that you have with you the applicants:

  • Name, National Insurance number, Date of Birth
  • Client contact details including a current phone number (if available)
  • Details of current & past accommodation
  • Full details of the applicants background including all known risks, offences and medications.
  • Full details of what support is needed and why it is needed

All questions marked with a * have to be answered. Please answer all the questions. If an application is incomplete or lacking in detail this will delay it being processed as we will have to contact the referrer to obtain more information.

We ask the question ‘Which of these describes you?’ – this is because the services are set up to work with particular client groups, and this, together with the ‘Support Needs Assessment’ part of the form, is used to help match you to a suitable service.
